Client Information:
My client provides specialise care packages to patients within the community, providing complex care packages in areas such as: acquired brain injury, spinal injury, palliative care, end of life etc.
Job Scope:
Reporting to the Registered Manager, you will oversee the clinical aspects of all care provision for clients to assist individuals to live within their own homes and commun! ities whatever their abilities.
This is a very hands ! on, operational role. You will take a lead with the Support Workers who are responsible for providing the day to day care of patients in the community. You will ensure Support workers are adequately trained, assessed, complete competencies, identify training needs whilst overseeing all care provided.
You will have a natural ability to form relationships with clients quickly and at ease, whilst remaining professional at all times.
This role will have a mixture of being office based and also attending to clients and patients within the community, therefore it is essential that you are willing to travel within the local area
Key Tasks:
Undertake comprehensive risk assessments
Undertake assessments for new packages of care
Review care plans and risk assessments on a regular basis
Promote Independence wherever possible
Review and monitor care packages
Develop and maintain relationships with clients
Manage client needs!
Manage the clinical aspects of the care in partnership with other healthcare professionals
Recruit any nurses required in the team
Interview, supervise, appraise and coordinate Nurses and Support Workers
Identify training needs of Nurses and Support Workers
Spot Checks
Assess competencies of the staff working in the packages
Establish & maintain Workers competency files ensuring required competencies are completed for all necessary workers.
Maintain ongoing relationship with workers, whether available to work or not.
Assists with ensuring branch's compliance with relevant quality policies and procedures and legislative requirements and keeping up to date with changes as determined by the Company, Clinical Governance and Quality and Compliance team.
Assists with ensuring all nurses and support workers cleared for work are compliant
Responsibility for answering incoming calls efficiently presenting a professional and businesslike m! anner at all times
Resolve clinical complaints/problems quickly & a! ccurately following appropriate procedure
Education / Qualifications
Registered Nurse with current band 6 / 7
Experience of working with ventilated patients / tracheotomy care
Experience within ITU / HDU / Community or District Nursing
Experience of undertaking comprehensive risk assessments
Ability to develop person centered care plans with clients
Excellent Interpersonal skills and communication skills
