If you are an experienced Diabetes Specialist Nurse with a passion for innovation and a track record of de! livering substantial change, then this newly created post offers a rare opportunity to lead and develop an integrated diabetes service with partner organisations across Camden.

The post of Diabetes Nurse Consultant will be hosted by CNWL but will have a primary focus on embedding integrated working across University College London Hospital NHS FT, Royal Free NHS FT, Central and North West London NHS FT and all primary care partners in order to deliver a population focused approach to diabetes across Camden.

We have set an ambitious plan to increase diagnosis and improve clinical competencies at all Tiers of Diabetes and implement care planning fully to ensure that people with diabetes are fully engaged and empowered to self-manage their diabetes to minimise complications. This role is pivotal to ensure all organisations involved in delivering diabetes care begin working as one team in a patient-focused, evidence-based way.

The post holder will pro! vide clinical leadership and operational management (in conjun! ction with the service manager) of the Camden Integrated Diabetes Service (CIDS). They will provide advice and support at the highest level for colleagues and patients in order to maintain clinical excellence, improve diabetes services, reduce the number of admissions, length of stay in hospital and to promote patient self-care and improved quality of life.

You must be a practising qualified nurse with a recognised diabetes qualification and have evidence of previous experience working at a Band 8 level.
