Description: Kent County Council is at present looking to employ Case Manager right now, this position will be placed in England. The Thanet Assessment & Enablement team has a vacancy for a Case Manager to work with older persons and adults with a physical disability to carryout community care assessments.
You will ! be qualified and registered in either Occupational Therapy, Nursing or Social Work.
You will be managing a caseload of clients living in the community as well as supporting hospital discharge planning. Good communication skills and the ability to be resilient and able to work under pressure when required. You will have a clear emphasis on promoting independence throughout the assessment process.
An understanding of the Mental Capacity Act and adult safeguarding procedures is desirable, but training will be made available.
Good time management and Information technology skills are essential.
