Band 7 District Nurse
£32,302 - £42,190 per annum inclusive of 5% HCAS
37.5 hours per week, including weekends
permanent post, base to be confirmed , will be within central surrey geographical area

CSH Surrey is a social enterprise healthcare provider established on 1st October 2006, providing healthcare to residents within East Elmbridge, Mid Surrey, Medlincs and Dorking area, commissioned by Surrey Downs CCG. Our aim is to provide first class integrated healthcare, working together with our patients, their families and health and social care professionals including voluntary/charitable organisations to provide ! better healthcare together.

We have vacancies in locations throughout areas covered by CSH Surrey for a qualified District Nurse Team Leader who will be able to manage and lead a team of Community Nurses and Health Care Assistants. The Team Leader is responsible for the delivery of community health services to provide effective assessment of care needs, diagnosis, care planning and evaluation of clinical effectiveness to patients within a domiciliary /residential setting. The District Nurse will effectively case manage and co-ordinate care. The Team Leader will work closely with General Practitioners and the wider multi-disciplinary team.

The candidate! will need to be motivated and innovative with strong leadership and co! mmunication skills to assist the Locality Team Leader in the development of the service.

The successful candidate will be a Registered General Nurse (RGN/RN)/NMC Registration and have the further qualification Certificate/Diploma/Degree in District Nursing and possession of nurse prescribing certificate.
