General Duties:
- To assess, plan, implement and evaluate all aspects of patient care, rehabilitation and treatment pathways.
- To act in a professional manner at all times and engage in the aims and objectives of the services.
- To supervise, support and carry out PDR's and 1 to1's for Assistant Practitioners / Generic Support Workers / Health Care Assistants working in the clinical area.
- To demonstrate a broad range of nursing knowledge, interventions and skills.
- To understand the principles of health promotion and preventing illness.
- To understand the roles of others in health and social care and e! nsure collaborative and partnership working.
- NMC registered
- Significant community nursing experience
- Significant medical rehab experience
- Strong leadership skills and ability to work autonomously
- Mentorship qualification is an advantage
- 27 days holiday per year, including bank / public holidays
