JOB D E SCRIPTION- SOCIAL CARE CO-ORDINATOR
Responsible to: The Home Manager
Accountability: The Home Manager/Director
PURPOSE OF POSITION: To work efficiently, conscientiously, safely, and supporting individuals with the procurement and fulfilment of all social needs .
SCOPE OF RESPONSIBILITIES
General Responsibilities
1. To contribute to the efficient operation of the home , a dher ing to home ’s policies and procedures.
2. To maintain both business and resident confidentiality.
3. To work as part of a team, key worker system and give support to other staff .
4. To ensure that any poor practice observed is reported to nurse in charge or the manager through the Whistle Blowing procedures identified in the home’s policy and procedures.
5. To take care of the home, its contents and consider the appearances of the home, ensur ing that the bedrooms and communal areas are tidy, clean and that the hygien e is maintained .
6. To answer residents bells and respond to emergency situations assisting the Nurse -! in-Charge.
7. To perform other related duties, not s! pecifically enumerated in this job description, which may from time to time be assigned by the home.
Activities Facilitation
1. To treat the residents individually with respect and preserve their personal dignity , respecting privacy and dignity at all times .
2. To enhance and promote the residents quality of life through the development of individual activities care plans which identify the service users needs in respect of social activity, achieved through effective assessment and consultation with the resident and possibly others involved in their care.
3. To work under the supervision of the Manager, liaising on issues of concern and also developing programmes of activity commensurate with the needs of the home at any given time.
4. To be caring, sympathetic and tactful when dealing with residents and relatives .
5. To assist residents when required or necessary in areas of feeding, and giving direction.
6. To p! romote physical, mental and social recreational activities and participate in them .
7. To report and record relevant information, relaying any concerns to the senior members of staff .
Personal Development
1. To undertake an induction training and address any issues bought to your attention therein or thereafter.
2. To participate in the supervision and appraisal process, identifying your own needs in relation to job role.
3. To contribute and participate in staff meetings .
4. To attend statutory and non-statutory staff training as required.
5. To ensure that you take personal responsibility for your actions and inactions understanding your level of accountability to the service user group.
