It is vital to ensure a smooth transition from! Hospital to home and particularly continuity of care across health, social and voluntary sector, in order to ensure care is maintained to prevent complications and a breakdown in care, which could result in a Hospital readmission.
The purpose of the âEarly Stroke Support Serviceâ is based around extending the current Information, Advice and Support service, by delivering an intensive co-ordinated hospital discharge service for all stroke survivors in the County Durham and Darlington locality.
We are looking for people with a background in Nursing, AHP, Social Work (or equivalent degree/related professional qualification), preferably with knowledge and experience of stroke, who must be able to communicate with people at all levels. A good telephone manner and administrative/IT skills are essential, and you must be a car owner/driver.
