School Nurse (SCPHN qualification essential)

Our vacancy
There is 1 post for 37.5 hours as a SCPHN School Nurse, South Devon.
Working primarily within the Teignbridge area.
Based at Newton Abbot Hospital but will require regular days at Lescaze offices, Dartington, on rota system to support PHN hub.
The Virgin Care Public Health Nursing team in Devon deliver an integrated 0-19 service for children, young people and families.
Team summary
You will join a nursing team based out of Ivybridge health centre and will w ork alongside fellow dedicated and highly skilled healthcare professionals. The team primarily covers the South Hams and Tavistock area but is part of a wider Southern locality team who work together to support equitable and safe delivery of the service.
Role summary
Our School Nursing team has a range of skill mix to support development and to enable you to deliver an excellent service locally. Virgin Care offers our School Nurses fantastic opportunities to further develop their career and we offer newly qualified School Nurses excellent preceptorship, mentorship and supervision.
The role will also involve:
  • Working within the team alongside Health Visitors and Community Health Workers. We have a great professional support network which includes regular interaction and team meetings that allows us to provide an invaluable service across a variety of settings
  • Undertaking holistic assessment and analysis to pl an, deliver and evaluate patient care
  • Delivering the School Nurse core offer and evidencing impact through data returns, collation of outcome measures, use of patient experience measures and case studies to demonstrate impact
  • application of leadership skills to work strategically to identify and address public health priorities
  • Implementing, monitoring and maintaining our high standards of care
  • Re-assessing and re-evaluating programmes of care when needed
  • Supporting our in-house mentorship training scheme with placements, students and staff
  • working within PHN hub to make clinical decisions on referrals and information received and being responsive to families and professionals making contact with the service.
  • use of electronic patient records
  • working with partner agencies to improve outcomes for children, young people and families. This wi ll include engagement in safeguarding processes, education health and care plans and early help plans.

You
The essential qualities we are looking for in our School Nurse (SCPHN) applicants are:
  • Entry on third part of Nursing and Midwifery Register as a Specialist Community Public Health Nurse as degree level or equivalent.
  • Training and Post - Registration experience
  • CPD Portfolio
  • Multidisciplinary approach to care
  • Excellent written and verbal communication skills
  • Excellent analytical skills to identify and assess risk/need and plan care accordingly
  • Excellent time management and organisational skills
  • Able to work under pressure, identify and articulate PHN role, prioritise workload, signpost, and challenge appropriately where necessary
  • Demonstrate ongoing professional development and a commitment to providing high quality, safe and effective care that meets the needs of patients
  • Leadership and supervisory skills to support the Community Health Worker and work strategically with school management teams and wider colleagues to identify and address public health priorities
  • Ability to relate to patients and colleagues in a sensitive manner
  • Ability and willingness to travel to home, school and clinic visits throughout our rural location
  • IT skills
