This is an exciting opportunity to take part in a pilot initiative to improve the health outcomes of young people aged 10 to 17 engaged in the Supporting (Troubled) Families Programme and working with the Youth Offending Team ! (YOT)/Youth Crime Prevention (YCP) team through delivery of th! e Healthy Child Programme (Universal and Progressive) and increased healthcare support.
The Youth Offending Team (YOT) is a statutory partnership made up of Social Workers, Police Officers, Probation Officers, Youth Support Workers and specialists in health, education, parenting, substance misuse and sports/arts. The YOT works with young people to prevent them from offending or re-offending and also supervises those who are already the subject of a Court Order.
You will be a Registered Nurse and hold a BSc/MSc Hons Public Health Specialist Practitioner (School Nursing) or equivalent (NMC Registered Part 3)
The pilot will focus on a geographical area (Basingstoke & Deane Borough Council area) and will be managed through the Youth Offending Team Health Team Leader on a secondment basis.
The role will be accountable to the Hampshire Youth Offending Team and the Local Coordination Group (LCG) who are responsible for the delivery of the Suppor! ting (Troubled) Families Programme in Basingstoke and Deane.
The Supporting (Troubled) Families Programme in Hampshire is an innovative partnership programme, aiming to provide better joined up and targeted support for some of HampshireÂ's most complex families and communities. Through working more effectively with whole families in partnership with a range of organisations across the public and voluntary sector, it provides support to families with young people involved in crime and anti-social behaviour, children not regularly attending school and adults out of work.
This is a fixed term contract until 31 March 2015 and a secondment from the NHS may be considered based on the equiavlent grade - Agenda for Change Band 6 - £25,783 to £34,530 per annum pro rata.
