The service supports the UK National Dementia Strategy, while offering personalised care packages for many other healthcare needs, including Parkinson's, multiple sclerosis and epilepsy.

You will be part of a multi-disciplinary team dedicated to providing high levels of care within an established care home for the elderly. This role will manage and co-ordinate care delivery and will be primarily responsible for promoting and delivering person-centred care, being mindful of resident’s age, gender, sexuality, ethnicity and spiritual needs. The post holder will work within a team ethos with the resident’s choices, dignity, independence and personal safety at the core of all clinical activity and will work within their professional ac! countability and competencies for their professional registrat! ion.

Registered nurse with current UKCC registration
Experience in care of the elderly
Practice knowledge of care planning
