WORKING HOURS: FT - 30 - 37 hours p/w, flexible on shifts.

THE ROLE: Practice Nurse in COPD and Asthma
An exciting opportunity has arisen for an experienced Nurse Practitioner to work for a leading practice in Louth Market Town.
The purpose of the role is to address all the medical needs of the COPD and Asthma patients in this practice.
Key Responsibilities to include: -
To deliver autonomous evidence-based patient care
Assess, diagnose, plan, implement and evaluate the patients respiratory needs
Be able to interpret spirometry results and liaise with GP's as appropriate
Assessing patients' respiratory needs referring them to other health care professionals as appropriate
Provide advice and treatment recommendations to the patient's specific respiratory needs
Promote health through empowering patients, offering a! ppropriate health education, information and advice
T! o support and facilitate smoking cessation initiatives
to document all interventions and related issues
To manage exacerbations and support discharges from secondary care
Manage and organise individual patient consultancies
Support others with their training and development needs
Follow all Health & safety rules relating to the practice Health & Safety and Infection Control policy
Participate and take responsibility of own personal development and training programs
THE COMPANY: Our client are a dispensing practice that have over 10,500 patients. This purpose built practice consists of 6 GP partners, 28 members of staff and offer many clinical and specialised clinic services such as general nursing clinic, phlebotomy, community nursings service, minor operations, GU clinic, well person screening and many more. This is an innovative practice who are always moving forward with the NHS regime, embracing new s! ervices being introduced by the NHS.
The Nurse Practitioner will be working with a small team of 6 doctors, 3 nurses and 3 health care assistants. The staff have proved excellent commitment and stability by serving the practice for many years. A really friendly and pleasant place to work.
THE CANDIDATE: Our client is looking for an experienced, qualified Nurse Practitioner who is a great team player. This would suit someone who is looking to settle within their career.
Key Experience/ Characteristics preferred for the role: -
Level 1 Nurse qualified
Clean UK Driving License with own vehicle
Qualification in Respiratory Nursing is desirable
Sound IT knowledge
Experience of working with SystmOne software is desirable
Excellent communication skills
Experience of working within Primary Care is desirable
Nurse Prescriber is desired but not essential
Well orga! nised and self-motivated
Must be flexible with their time
