To be responsible for the case management of patients in a defined area, working closely with the District Nurse and other members of th! e multi-disciplinary teams. Undertaking patient needs assessment as required, to ensure appropriate access and use of MCCC services.
To be responsible for the prioritisation of care for a defined group of patients according to their need ensuring they receive the right care in a timely fashion.
To support the Clinical Lead Nurse to ensure that any nursing care that is provided is to the highest possible standard for all patients and their families, which promotes their wishes to recevie care in the community.
To support the Clincial Lead Nurse to support and motivate the nursing team to drive up standards of care and facitlitate growth in knowledge and skills.
To undertake work as a Marie Curie Nurse (MCN), providing nursing & clinical care to patients and families â€" the proportion of time between the direct care element of the role & the clinical case management element will vary weekly, in order to provide the flexibility to meet the ! ever changing needs
