Further informations about this vacancy opportunity please give attention to these descriptions. We are looking for an experienced and dynamic Clinical Nurse Specialist in Diabetes. You will play a pivotal role in working collaboratively with a range of multi disciplinary professionals and teams across the Trust and in Primary Care.
This team is dedicated to promoting liaison with acute services to accelerate hospital discharges and prevent hospital admissions by working across organisational and service boundaries.
You will lead on the provision of specialist advice and guidance and act as an expert resource to all health care professionals caring for patients with diabetes.
Key elements of the role will include leading the development of a structured diabetes pathway and developing a nurse led service in the monitoring and assessment for patients with diabetes.and forming strong links across organisational boundaries.
Following a successful pilot project you will also lead on the proposed structured roll out of a world class diabetes service across the whole organisation
You will work closely with the diverse medical, pharmacy and nursing teams to deliver on targets related to the NICE/NSF objectives and efficient and effective treatment plans for patients.

Experience of developing structured group and one-to-one education and training is highly desirable. The post requires the applicant to manage a case load autonomously, but also lead a team, contributing to multi-disciplinary team decisions about the individuals’ personal plan of care
The post holder will work across departmental, divisional and Trust boundaries to support the delivery of a patient centred seamless service.
You will need to demonstrate excellent communication and interpersonal skills in order to network across organisational and professional boundaries.
You will need to be educated or working towards Masters level within the relevant specialty or be able to demonstrate a considerable equivalent level of experience working within the acute care sector.
You will have a proven track record in the specialist care of diabetes for inpatients and will be able to demonstrate effective leadership and managerial skills to enable you to optimise our service delivery and take practice forward for these patient groups.
