Social Care Managerâ€" Hospital Discharge Team 27 Hours per week (Fixed term / secondment till 31 st July 2013) A secondment / fixed term contract opportunity has arisen for a Social Care Manager to jointhe busy and well established Hospital Social Work Team based at York Hospital. This vacancy is available as a secondment opportunity, candidates must obtain permission of the manager of their current post if applying on a secondment basis. The successful candidate will be able to work collaboratively within a multi disciplinary setting, ensuring that a social care perspective is offered. You will be able to manage your time and workload effectively, using organisational, assessment and administrative skills.
Experience of hospital social work and care management would be benefit, although are not essential. Applicants for this postare required to hold a relevant professional qualification of:- Social work, e.g. C.Q.S.W., C.S.S., Diploma in Social Work (DipSW ) or Nursing, Occupational Therapy or similar as defined in the National Conditions of Service and a minimum of 2 years’ post qualification experience, including a minimum of 1 year’s community based practice.
