Our client provides nationwide specialist healthcare services to patients at home across a wide range of therapies, where they are committed to delivering safe, effective high quality care to their patients.
We are currently recruiting for a Complex Care Nurse to cover either East or North East London region. Working as part of team of Paediatric Nurses you will manage a designated case load of patients.
This role will be field based and there is an expectation to travel within the region to delivery services and on occasions you will be expected to travel outside the region in line with the business needs.
The role will include:
To demonstrate excellent communication skills with patients, families and other health care professionals including PCTs and other Commissioners of services. To keep clear and accurate care records including the management of sensitive and confidential information Discuss, produce and update individuals care plans with the care team, patient and family Understand how the long term physical, psychological and social impacts of a specific diagnosis and treatment affect an individual patient Escalate to the line manager any changes in an individual's health and well being or information that requires reporting Maintain confidentiality in line with Data Protection legislation, knowing what information can be disclosed and the process for ensuring this meets the Clinical Governance and Information Governance needs.
Qualifications and training Qualified RSCN or RGN NMC registered Community Care qualification, would be a distinct advantage City and guilds 730 or ENB 998 or equivalent desirable.
Knowledge / Experience Community care experience would be beneficial but not essential. IV Therapy skills Phlebotomy skills desired although training will be given Some management or training experience would be a distinct advantage to oversee the care team Experience in complex care, especially ventilators and tracheas essential. Basic IT skills Ability to communicate verbally and in a written format at all levels within the organisation Ability to work independently off own initiative. Car driver with clean driving licence Travel and working flexible hours necessary
